Because smaller incisions are used than in traditional knee replacement surgery, the joint is exposed far less. This translates into less pain and trauma. A patient who has a successful minimally invasive knee surgery procedure can expect to be hospitalized a very short time and have a quick and easy recovery period.

This surgery is recommended for the patient who is of an average weight and activity level. The patient should have fairly good ROM (range of motion) and be relatively free of knee deformity. A patient who meets all of these criteria and has a successful procedure will be hospitalized for 3 days or less and will have little or no inpatient rehabilitation process.

In general, these are the accepted findings. It is worth noting that these findings are not entirely consistent. Some studies show dramatic reduction of pain, hospital stay, and rehabilitation. Others show that these aspects of recovery are quite similar to those experienced with standard knee replacement surgery.

Additionally, the risks of this method of surgery have not truly been documented; however, there is no reason to believe that they are substantially different from those associated with any knee surgery. This is a topic you will want to discuss with your orthopedic surgeon.

Time At The Hospital

You will stay at the hospital for a short time after your surgery. During this time, you will be given medications to handle any pain you may be experiencing.

Be sure to discuss your medication regimen with your orthopedic surgeon in advance of your surgery. You do not want any surprises. This is a very important part of your recovery, so it should go smoothly. Also, be prepared to begin moving your knee right away after surgery. This is also quite important to your overall recovery.

During your evaluation, your orthopedic surgeon will talk with you and do a few tests:

First, a medical history will be taken that covers your health in general and your knee symptoms in particular.

Physical Examination: To assess the strength, stability and ROM (range of motion) of your knee as well as the alignment of your leg.

X-rays: Radiographs will be taken to verify the amount of deformity and damage that are present.

A variety of tests, including MRI (magnetic resonance imaging), blood tests, and bone scan will be conducted to determine the condition of the soft tissues and bones of your knee as well as your general physical condition.

Another option is to receive an allograft. This is a graft of tissue harvested from a deceased person.

There are both advantages and disadvantages to the use of each type of graft. Your surgeon will determine which type will work best for you.

ACL (anterior cruciate ligament) is usually performed using arthroscopy. This is a type of surgery that utilizes a miniature camera to image the interior of the knee. The camera is introduced into the knee chamber via a poke-hole or small incision. This method lets the surgeon get a clear view of the knee chamber.

Your surgeon will examine the ligaments and the cartilage in your knee. If he or she finds that there has been other damage (for example, a meniscus tear) it will be taken care of during your surgery.

Several types of anesthesia are used for arthroscopic knee surgery, but you will probably receive general anesthetic. This will allow you to sleep during the surgery. While you are sleeping, your surgeon will replace your ACL (anterior cruciate ligament).

Your surgeon will also make a few other small incisions around the knee. These will allow the surgeon to place your new ligament properly. Your damaged ligament will be taken out with a shaver or some other instrument. The exception to this is that, if you will be using an autograft, a larger incision will be needed to remove the tissue that is to be used for grafting.

Your surgeon will create bone tunnels to be used in placing the new ligament in the exact location of the old ligament. Once the bone tunnel is in place, your new ligament will be positioned and attached to the bone with screws or some other form of fastener. This will hold the ligament in its proper place. At the end of surgery, your surgeon will close your incisions and apply a bandage.

Your surgeon will probably take photos and/or film your operation so that you can watch it on the video monitor after your surgery is done. This technique allows the surgeon to discuss findings with you in detail.

If you have unstable knees, pain in your knees, or your knee gives out on you unexpectedly, you may be referred for ACL (anterior cruciate ligament) reconstruction. Additionally, if you are simply unable to play sports and/or participate in ADL (activities of daily living), ACL (anterior cruciate ligament) reconstruction may be the answer for you.

As with any surgical procedure, there are some risks involved. Excessive bleeding, nerve damage and infection occasionally occur. Patients also report weak knees and pain and stiffness in the knees. Sometimes, the surgery does not resolve the symptoms. Occasionally, the ligament does not heal.

Chronic knee pain and disability are often caused by arthritis. There are three very common types of arthritis that cause knee problems. They are: rheumatoid arthritis, traumatic arthritis, and osteoarthritis.

Traumatic Arthritis: Injury causes traumatic arthritis. A fractured knee or torn ligaments cause excessive wear and tear on the knee causing the articular cartilage to break down. This painful condition can only become worse with the passage of time.

If the synovial membrane of the knee produces too much synovial fluid, it will over-fill the joint space. The fluid will thicken and become inflamed, and it will damage the knees cartilage. This will cause stiffness and pain and a breakdown of the cartilage. This is rheumatoid arthritis.

If the cushion of cartilage in the knee wears out, it leaves the bones to grind against each other. This extremely painful condition is known as osteoarthritis. It is usually a problem for people over fifty years old, especially those who have a history of osteoarthritis in the family.

Knee Anatomy – A Short Lesson

The shin bone, the knee cap, and the thigh bone are the main components of the knee. In medical terminology, these are called the tibia, the patella, and the femur. Your knee is in almost constant use, and it is the largest joint in your body. For these reasons, you are highly likely to injure your knee or experience some wear and tear at some point in your life.

Four ligaments attach the femur and tibia and support and stabilize the knee. They are the anterior and posterior cruciate ligaments and the medial and lateral collateral ligaments.

The muscles of the thigh also play a strong part in the strength and stability of the knee. Strong thigh muscles mean strong knees.

Articular cartilage forms a pad between the bones of the knee. This pad keeps the bones separated and allows them to move without grinding against each other. With this smooth cushioning your knee can move smoothly and easily. Additionally, the knee comes equipped with natural shock absorbers called the lateral menisci. These are semicircular, fibrous cartilage rings that add stability to the overall structure.

All of the bones of the knee are lined with synovial membrane. This thin, smooth tissue makes a special lubricant that keeps all of the parts of the knee operating smoothly.

All of these parts should work well together and give you a lifetime of painless service. However, because the knee is the most used and most often injured joint in the body, this is often not the case. An injury or illness such as arthritis can throw this delicate balance off. This can cause you loss of mobility and strength, as well as a great deal of pain.

The usual candidate for total knee replacement surgery is between the ages of sixty and eighty; however, this is not always the case. Many very young patients and many very old patients have also had successful knee replacement surgery for everything from juvenile arthritis to degenerative arthritis to injury. When your orthopedic surgeon evaluates your case, he or she will look more at your pain and your loss of mobility than at your age.

There are a few physical differences between your new knee and your old knee. For one thing, you may feel a little numbness and stiffness around your new knee. Excessive bending may tend to increase the stiffness. Your new knee may not have quite the ROM (range of motion) your old knee had. Nonetheless, you are sure to find your new knee is an improvement over your old knee.

You may also notice that you are having some trouble kneeling. Dont worry about it, though. It does not hurt your new knee to kneel. You may also hear clicking sometimes when you are bending and walking. This is just the metal parts moving against the plastic. It will become less with time. Most patients adjust to it quite nicely. It is surely less of a problem than the pain and restriction of activities experienced with a diseased natural knee.

After your surgery, your activity level will be determined by your ideal pre-surgery activity level. If you were active when your natural knee was well, you can be active again. Your new knee will bend and straighten to allow you to get in and out of cars, to climb stairs, and perform other activities of daily living.

When you go to the airport or a federal building or other building with a metal detector, be sure to have a word with the guards to avoid unpleasant incidents. Your new knee has some metal parts that will set off the alarms.

Trust your orthopedist. He or she is also a medical doctor. Therefore, a lot of training backs up his or her diagnosis and treatment plan. Your orthopedist is familiar with the musculoskeletal system (bones, nerves, joints, muscles, ligaments, and tendons) and fully capable of determining both surgical and non-surgical treatments.

When you go to the dentist, be sure he or she knows about your knee replacement. You must always take antibiotics prior to any dental procedure to prevent bacteria from traveling to the site of your operation and causing problems.

Be sure to participate in light exercise on a regular basis to maintain the mobility and the strength of your new knee.

Visit your orthopedic surgeon every year. He or she will want to follow up with x-rays and an annual examination.

If you tear your ACL, your knee may give out while you are engaged in physical activity. If you dont have this treated, your knee may give out time and again. This can cause early arthritis as well as damage to the cartilage.

You can have this torn ligament replaced with ACL reconstruction. This surgery replaces your damaged ligament with a new ligament. Your surgeon could get a new ligament from you (autograft) or from a deceased donor (allograft).

If you decide to use an autograft, the tissue will probably come from your hamstring or the front of your knee (your patellar tendon).

Your surgeon will probably use arthroscopy to perform your ACL reconstruction. This type of surgery is usually done using general anesthesia.

Arthroscopy is conducted with the assistance of a small camera inserted into a joint. Your surgeon will make a small incision called a poke-hole to insert the camera. Then the surgeon can get a clear view of the inside of your knee on a video monitor. This will allow your surgeon to check the condition of all of the structures of the knee. If your surgeon discovers damage in addition to your ACL tear, he or she will go ahead and fix the problem.

There will be a few other incisions in addition to the poke-hole. First, if you are using your own tissue, an incision will need to be made to remove it. This is rather a larger incision. Then a few smaller incisions will be made to access and remove your damaged tissue. Your new tissue will be inserted via bone tunnels. It will be placed perfectly and secured with screws or a variety of surgical fasteners as appropriate.

When your surgery is finished, your incisions will be closed, and a bandage will be put on. When you wake up from your anesthesia, you will probably be able to see pictures and the video of your surgery. Your surgeon will discuss the procedure with you and answer your questions.

Patients who have a chronic illness will be a bit more likely to experience complications. Rehabilitation may be extended by chronic illness, and complete recovery may be hindered.

Clotting of the leg veins is the most common complication experienced by patients; however, this can usually be avoided by following doctors orders. Your orthopedic surgeon will give you a complete list of instructions following surgery. They may include the use of compressive stockings, taking blood thinning medications, performing specific exercises, and keeping the leg elevated at intervals. Be sure to follow these instructions, and the incidence of blood clots will be greatly decreased.

Sometimes a part of the replacement may wear or loosen. This is quite unusual because the parts of the replacement are very well made, and the surgery is performed very carefully.

Although a patient can usually expect to have a ROM (range of motion) of 115 after surgery, there are some exceptions. For example, if you had very limited ROM (range of motion) before the surgery, it will be quite likely to be somewhat limited after the surgery. Additionally, on very rare occasions, damage to the blood vessels or nerves of the knee may occur. This can limit motion somewhat.

What to Expect During Home Recovery

The vast majority (over 90%) of fixed unicompartmental as well as mobile bearing knee replacements function well for as much as ten years. The Care of Your Wound

Your orthopedic surgeon may close your wound in one of three ways. He or she may staple or stitch it shut exteriorly or close it with subcutaneous sutures. If the wound is closed with interior sutures, they will dissolve, and you will not need to do anything.

Keep your wound dry until it is completely healed. You can place a bandage over it to protect it from rubbing against your compression hose or your clothing.

Exercise Your Knee

You will be sent home with a list of exercises that are intended to facilitate your speedy recovery. It is essential that you do them. You may feel a bit of pain while doing them; however, slight pain is normal and nothing to worry about. You may also feel a bit of pain while resting at night. As you heal, this will pass, and you will be back to normal and able to resume your regular daily activities in a month to six weeks.

For example, if you have injured your knee(s) through hard use, like jogging, there will probably be a lot of damage to repair. Your articular cushion – the weight bearing part of your knee – may be badly degraded. If this is the case, you may never regain full use of the knee. After your surgery, you will surely need to limit your exercise regimen to low-impact choices.

You can speed your recovery time by following your doctors orders to the letter. Following your physical therapy routine exactly will help your knee to recover quickly and efficiently.

Another thing that will have an impact on the outcome of your surgery is your physical condition. People who are in better condition to begin with, recover more quickly. So you can see that it is not realistic to compare your recovery to that of a professional athlete, even if you have the same knee injury. The athlete will surely recover more quickly because his or her knee muscles will probably be in better condition.

Understand that, even if you have been a fairly serious athlete prior to your injury, it will be necessary for you to modify your fitness program to avoid re-injury.

Expect to devote 4-6 weeks to your complete recovery from arthroscopic knee surgery. While you should be able to get back to your day-to-day tasks pretty quickly, understand that returning to your exercise program will take time and modification.
